I am having a VERY similar issue.

Were you able to resolve this?

Also, I narrowed it down to U.ID being null. There for auth is failing.

Any idea why? it works sometimes and other it does not?

I don't know the way this happen, but i know how to solve this, You cannot have "offline_access" on your ExtendedPermissions , if you registered a app you have to remove and set the permissions again without "offline_access".

"publish_stream" is supposed to supercede "offline_access", but there doesn seem to be a bug in the FGT - when using "offline_access" on a SocialPage you get a redirect loop
